Office visits are self-pay. Insurance is not accepted for office visits. Payment is due at the appointment. Call 520-326-7246 for current consultation fees.
Office hours: Monday-Friday, 9 AM-4 PM. Closed Saturday and Sunday.
Records Required Before Your Appointment
Ask prior providers to fax records from the last 3 visits of pain management care, most recent x-rays and MRI's, and the most recent pain-management documentation to 520-844-7092 before your appointment.
