To help your provider evaluate your care appropriately, please arrange for relevant medical records to be sent to Canyon Health before your appointment whenever possible.

Ask prior providers to fax records from the last 3 visits of pain management care, your most recent X-rays and MRI reports, and the most recent pain-management documentation to 520-844-7092 before your appointment.
