Records Needed Before Your Pain Management Visit
Providing complete and current records helps our clinical team better understand your treatment history and may help avoid delays in the new-patient review process.
Three Most Recent Pain Management Visits
Please request records from your current pain management provider or from the healthcare professional who is currently prescribing your pain medication. We ask for documentation from your three most recent visits.
Current X-Rays and MRI Reports
Please provide all current and relevant X-ray and MRI reports related to the condition or pain concern for which you are seeking treatment. Additional relevant imaging or specialist records may also be helpful.
Send Records to Canyon Health
Your healthcare provider or medical-records department may fax the requested records directly to Canyon Health.
