Do I Need Previous Medical Records Before a First Pain Evaluation?

Starting with a new clinician is easier when the care team can see the story behind your symptoms. If you are preparing for a first visit at Canyon Health, previous medical records can be useful—but you do not need to arrive with a perfectly organized file.

The goal is to give your clinician enough context to understand what has already been evaluated, what has helped, and what has not.

Which records are most useful?

If they are available, consider bringing or arranging access to the records that best explain your current concern. These may include:

  • Recent notes from clinicians who have evaluated the same pain problem
  • Imaging reports such as X-ray, MRI, or CT results
  • A current medication list, including doses when known
  • A brief history of past surgeries, injuries, or hospital care related to the problem
  • Physical therapy or rehabilitation records when they are directly relevant
  • A list of medication allergies or reactions

You usually do not need every medical record you have ever had. A smaller group of relevant, recent records is often more helpful than a large stack of unrelated paperwork.

Should I bring the actual imaging or just the report?

An imaging report can provide important information, but sometimes a clinician may also want access to the images themselves. If you have a disc, portal access, or another way to share the images, bring that information with you. If you are unsure what is needed, contact the office before your visit so the staff can tell you what would be most useful.

What if I cannot get my old records before the appointment?

Do not assume that you must cancel simply because a record is difficult to obtain. Contact the office and explain what is missing. Depending on your situation, the visit may still be useful, or the staff may recommend obtaining specific records first.

It also helps to write down what you remember: when the pain began, what seems to make it better or worse, treatments you have tried, and how the problem affects sleep, work, walking, exercise, or other daily activities.

Why do previous records matter?

Relevant records can help a clinician understand the timeline of your symptoms and reduce unnecessary repetition. They may show which tests have already been completed, what treatments were tried, and whether your symptoms have changed over time.

Records are only one part of an evaluation. Your current symptoms, medical history, physical examination, and goals for daily function are also important.

A simple way to prepare

Before your appointment, create one short list with your current medications, major prior treatments, important imaging dates, and two or three questions you most want answered. That can make the visit more focused even if some outside records have not arrived yet.
